Field Care Manager
RCM is seeking RN's care managers to join our team.
We are partnering with a leading Care Management Network to assist with coaching, coordinating and arranging care management services to their insured members. As a member of the team, you will have the opportunity to make a impact on the lives of people living with complex and/ or chronic conditions. Care Managers come in contact with the insured member in person, on the phone or in the hospital. The goal is to assistance the member (s) with resources to improve and maintain good health and coach them to better understand their illness.
Qualifications:
Nurses with Care Management experience.
Required:
Own a updated computer.
Good driving record.
At least 3-5 years of nursing experience, preferably in home health, ambulatory care, community public health, case management or care management.
Current Nurse license in good standings.
Willing to travel with Greensboro and surrounding areas.
Need to be reliable and dependable.
Preferred:
We are seeking team members who possess enthusiasm, integrity, good work ethics and enjoy collaborative experiences.
Knowledge and experience teaching self-management.
A current North Carolina Registered Nurse License in good standing.
Strong written and oral communication skills.
Provides compassionate and respectful care.
Ability to meet hard deadlines.
Ability to use computers and EHR.Estimated Salary: $20 to $28 per hour based on qualifications.
